At this time DHA is serving only members and their dependents of UFCW Food Division Locals 770/1036 and 324 who are enrolled in the Indemnity Dental Plan. To confirm which dental coverage you have and if you are eligible call your Union Local (see "Contact Us" tab at top the page).

Remember that patient eligibility is month to month and is based on the hours worked two months prior to the current month (i.e. the hours worked in March determine the eligibility for May).

Emergency Care

If this is an emergency immediately log in with your username and password and open the link on the desired city. A telephone number in case of an emergency will be provided for each office. Relief from pain is available 24 hours a day, seven days a week.

Non-Emergency

DHA has over 150 dental offices to serve you. Find a dentist convenient for you by logging in with your user name and password at the top of this page.

Costs

Your union local has negotiated quality dental care with little or no co-payment for your dental services when you choose a DHA dentist. All plans have an annual deductible per calendar year of $50 per person with a family maximum of $150. The deductible does not apply to routine preventive and diagnostic care (cleaning, examination, and x-rays). Some offices may waive all or part of the annual deductible. Please contact the dental office directly for deductible and co-payment information. Depending on which dental plan you qualify for there will be a maximum that the union will pay. Once you reach your yearly maximum additional payments will be required.

Your union local has also negotiated with DHA a reduced fee if you need to see a specialist, root canals (Endodontist), gum disease (Periodontist), children's dentistry (Pedodontist), or an Oral Surgeon. Their services will require additional co-payments.
